Hospice Medical Social Worker

Our client is seeking a compassionate Medical Social Worker to join their Hospice care team in West Branch, MI. This position provides an opportunity to make a meaningful impact on patients' lives through collaboration, emotional support, and care coordination. You will work closely with an interdisciplinary team to manage patient care plans and provide guidance to patients and families throughout their care journey.
Responsibilities include managing and delivering patient care plans, supporting patients and families in West Branch and surrounding areas, maintaining accurate documentation, and managing time and travel effectively.
Schedule: Monday Friday, 8am 5pm, with rotating weekends
Travel: Local travel within assigned region required
Required qualifications:
Master's degree in Social Work from an accredited program (CSWE-approved)
One to two years of post-graduate experience in a healthcare setting
Active MSW license in the State of Michigan
Valid driver's license, reliable transportation, and auto insurance
Experience using EMR systems such as TruChart
Ability to meet charting deadlines and manage time independently
