Geriatrician

Geriatrician is needed in New Haven, CT.

Responsibilities

• Monday – Friday (8:00am – 5:00pm)
• Collaborate with support staff to ensure high-quality care
• Deliver comprehensive primary medical care to adult and geriatric patients (ages 60+)
• Conduct thorough patient assessments including medical history, physical exams, diagnoses, and personalized treatment plans
• Utilize diagnostic tools such as labs and imaging in a cost-effective, clinically appropriate manner
• Collaborate with interdisciplinary teams, including clinicians, families, and support staff, to coordinate patient care
• Initiate specialist referrals when needed while maintaining responsibility for continuity of primary care
• Maintain up-to-date, accurate medical documentation and ensure timely follow-up with patients
• Actively participate in medical staff meetings, clinical team discussions, and organizational committees
• Contribute to quality improvement initiatives and engage in ongoing professional development to enhance clinical knowledge and care standards

Requirements

• Current and active Internal Medicine license in the state of Connecticut
• Certified as Geriatrician
